You need to plan ahead for your safety net.</p><h2>Plan ahead for healthcare</h2><h3>BEFORE GOING TO THE HOSPITAL</h3><p class="">What do you do if you get sick or need to go to the hospital? First, <strong>have a plan in place before you get sick</strong>. <strong>In an epidemic, the hospital may seem to be a chaotic place.</strong> You may be in an temporary isolation tent or placed in a hallway. <strong>Being prepared ahead of time is <em>key. </em></strong></p><p class="">Call your doctor’s office to navigate their phone menu. See if their office will give you a direct line to a nurse. Many offices now have apps to bypass the telephone and contact their office directly.</p><h3>AT THE HOSPITAL</h3><p class="">If you must go to the hospital, know that hearing loss can increase the risk of poor treatment. It is essential to <strong>bring a friend or loved one to act as your advocate</strong>. You also need to <strong>bring all your hearing equipment with you</strong>. Keep your hearing equipment in your ears to prevent loss. In the best of times, hospitals are prone to medication and treatment errors. This will worsen during a busy pandemic. It is essential that you and your advocate be aware of medication and treatment plans. See this <a href="https://www.consumerreports.org/cro/2012/10/your-hospital-survival-guide/index.htm" target="_blank" rel="noopener">hospital survival guide</a> for tips before going to the hospital. It includes tips such as bringing a list of your medications and creating a living will so family members understand your wishes. This <a href="http://nwadacenter.org/audience/healthcare-access" target="_blank" rel="noopener">healthcare access toolkit</a> provides information for healthcare providers and for individuals with disabilities about accessible healthcare.</p><h3>PLANNING TO GO HOME</h3><p class=""><a href="https://www.consumerreports.org/cro/2012/10/your-hospital-survival-guide/index.htm" target="_blank" rel="noopener">Plan for your discharge</a> from the hospital while you are in the hospital. This is essential for being discharged as quickly as possible to avoid hospital-transmitted infections. Contact the social worker on your hospital ward to get this process started. Healthcare providers will be wearing masks, and you will likely be in respiratory isolation. This can make communication challenging. Some ideas:</p><ul data-rte-list="default"><li><p class="">Use a personal whiteboard or a <a href="https://myboogieboard.com/products/jot" target="_blank" rel="noopener">boogie board</a>.</p></li><li><p class=""><a href="https://www.safenclear.com/">Clear face masks</a> and orthopedic surgery hoods (see figure) are types of clear surgical masks. Clear face masks must be purchased. For ortho hoods, ask for the charge nurse of your ward or hospital nursing supervisor to contact the operating room staff or an orthopedic surgeon to borrow a hood.</p></li><li><p class="">Apps such as <a href="https://play.google.com/store/apps/details?hl=en_US&amp;id=com.google.audio.hearing.visualization.accessibility.scribe">Google LiveTranscribe</a> and <a href="https://www.microsoft.com/en-us/translator/">Microsoft Translator</a> can be backup communication aids that provide captions for speech. <a href="https://www.amazon.com/Professional-Microphone-Omnidirectional-Condenser-Livestream/dp/B07Q6BF9DG" target="_blank" rel="noopener">Wired</a> or <a href="https://www.kickstarter.com/projects/117828565/smartmike-revolutionary-wireless-mic-for-content-creators" target="_blank" rel="noopener">wireless</a> lavalier microphones can increase accuracy. Wired mics are easier and less prone to being lost in the hospital.</p></li><li><p class="">You have the <a href="https://www.nad.org/about-us/position-statements/position-statement-on-health-care-access-for-deaf-patients/">right</a> to a <a href="https://drive.google.com/file/d/0B3DKvZMflFLdYVVBd0RIWDlOMW8/view?fbclid=IwAR230XvF-HeAlDTHITUqYkao-VM85yC2A5eT7p_x3ZO303HvxG6t6cNban8" target="_blank" rel="noopener">live ASL interpreter</a> and if not available, <a href="https://www.nad.org/about-us/position-statements/minimum-standards-for-video-remote-interpreting-services-in-medical-settings/">quality video interpreting services</a>.</p></li></ul><h2>Getting a Cochlear Implant During Pandemic COVID-19</h2><h3>COCHLEAR IMPLANTATION IS GENERALLY “ELECTIVE”</h3><p class="">In general cochlear implant surgery is elective or non-urgent surgery. Given the seriousness of COVID-19, you should strongly consider rescheduling your surgery. It may be some time (e.g. a year) before you may get your surgery. Some patients may be concerned about longer durations of deafness since this has been shown to worsen cochlear implant outcomes. Fortunately, the short period of time to wait for COVID-19 to pass should not have an appreciable effect on cochlear implant outcome. This is because “long duration of deafness” is defined as longer than 10 years since hearing aids were beneficial.</p><h3>WHEN IS COCHLEAR IMPLANTATION URGENT?</h3><p class="">In two instances cochlear implantation is not elective and surgery must proceed urgently. For patients with meningitis, bony growth (ossification) start to quickly block the inside of the cochlea. The inside of the cochlea must be free of bone so that the electrode can be successfully placed. The second instance is congenital hearing loss. Children born with hearing loss who are from hearing families who do not use American Sign Language are at very high risk for language delays. Cochlear implantation as early as possible has been shown to improve language outcomes. 									<p><strong>Ototoxicity</strong> refers to a poisoning of the ear, caused by prescription or over-the-counter drugs or substances. Currently, there are approximately 200 classes of medications that are considered ototoxic.</p><p>Ototoxicity occurs as a side-effect of certain drugs and substances, which damages inner ear hair cells and has been known to cause sensorineural hearing loss, dysequilibrium (balance disorders), or both. Depending on the strength of the drug and its side-effects, hearing loss or other disorders may be temporary or permanent.</p><p>Beyond prescription or over the counter medications, many people slowly damage their hearing with common substances such as caffeine and alcohol. While the impact of these substances is more gradual over the course of many years, the resulting hearing loss is typically permanent. Changes to a person&#8217;s lifestyle earlier in life can dramatically reduce one&#8217;s risk for developing sensorineural hearing loss.</p>								</div>

									<p>Sensorineural hearing loss is one of three types of hearing loss and relates specifically to the structures of the inner ear.</p><p>After sound waves have made their way from the outer ear through the middle ear, and the waves have become vibrations created by the eardrum, the vibrations move through the inner ear structure, where they are picked up by inner ear hair cells. These cells are responsible for translating sound wave vibrations into neural signals that are processed and recognized by the auditory cortex of the brain as sound.</p><p><strong>Ototoxicity causes the death of these inner ear hair cells, which do not regenerate. This leads to sensorineural hearing loss.</strong></p><p>Hearing loss should be treated as soon as possible; if left untreated for an extended period of time, the neural pathways that process sound in the brain will fall into disrepair and will make treating hearing loss more difficult in the long term.</p><p> </p>								</div>

									<p><strong>Aminoglycoside antibiotics</strong> are a strong and effective treatment for inflammatory bacterial infections such as meningitis, bacteremia, and respiratory diseases in patients with cystic fibrosis.</p><p>At the Oregon Hearing Research Center, Dr. Peter Steyger and his colleagues tested the effects of aminoglycoside antibiotics on mice, one group healthy and the other infected with inflammatory bacteria.</p><p>They found that both groups experienced levels of hearing loss, with the infected mice experiencing a higher degree.</p><p>Aminoglycoside antibiotics are frequently used in developing countries with warm climates due to their ease of storage (they do not require refrigeration). They are also used in the US, where they are administered to infants in intensive care units.</p><p>Though aminoglycoside antibiotics are life-saving in cases of lethal infection, they may cause permanent hearing loss.</p><p> </p>								</div>

At the core, it is because hearing loss affects our abilities to recognize speech and communicate. When clear communication is the foundation of healthy relationships, this breakdown could lead to a rift in marriages, friendships, and family relationships. For this reason, it is important to encourage a loved one to seek treatment for hearing loss.  </p>								</div>

The configurations are different for everyone. As hearing loss affects the way people recognize speech, so it is important to choose a quiet time and place to have the conversation. Additionally, because hearing loss may be a sensitive topic, you may want to find a private place to have the conversation. With hearing loss, a tell-tale sign is when someone asks you to repeat what you say. Make sure that you are speaking clearly when speaking your loved one. Volume is not necessarily the issue; rather, focus on enunciation. Make sure you are facing your loved one when you speaking. A well-lit room is also important.  </p>								</div>

For example, Johns Hopkins University has conducted a series of studies that show the link between untreated hearing loss and a higher risk for developing dementia. Untreated hearing loss could lead to an increased risk for falls and accidents, since we depend on our sense of hearing to keep us safe. Most importantly, hearing loss affects our relationships with each other. Your concern shows your investment in your relationship with your loved one. Encouraging them to take a hearing test is the first step to treating a hearing loss that could otherwise create a rift in your relationship. Offer your loved one support as they journey to better hearing health.  </p>								</div>

In the US, one in three people between the ages of 65 and 74 experience some degree of hearing loss. For people over the age of 75, 50% have a hearing loss. For people age 85 or older, 80% experience some degree of hearing loss. With presbycusis, people usually experience hearing loss in both ears (bilateral hearing loss) and because it occurs gradually, people do not often recognize that they are experiencing hearing loss. <em> </em></p>								</div>

Given the high percentage of Americans age 65 and older that experience hearing loss, “primary care physicians routinely screen only about 12.9% of the total US population for hearing loss.” Dr. Justin Golub, an otolaryngologist at Columbia University Medical Center in New York, says, “It’s a fact of aging that even baby boomers will face hearing loss. Yet, fewer than 1 in 5 people will treat age-related hearing loss. This statistic has not changed in over 40 years.”  </p>								</div>

Hearing specialists recommend that people begin taking an annual hearing test from the age of 50. 									<p>To understand how listening to music frequently and at high volumes can harm your hearing, one must first understand how we actually hear.</p><p>First, sounds come in through the ear canal where they are met by delicate and microscopic hair cells that convert sound vibrations into electrical signals. These electrical signals are then sent to our brain for processing, where our brain coverts the signals into meaningful sounds. It’s a pretty impressive system, especially considering the entire process happens in a matter of nanoseconds. Noises at decibels 85 dBA or higher can cause permanent damage to the delicate hair cells that play such an important role in our hearing. Once these hair cells are damaged, they can never be repaired, once they are gone, they are gone.</p><p>Combine this damage with the fact that earbuds sit so far into the ear canal and blast these sounds directly into the eardrum, it becomes easier to understand how they can be considered so dangerous.</p>								</div>

									<p>You don’t need to trash your earbuds and revert to listening to records in order to protect your hearing. There are many steps you can take to ensure you are listening to your earbuds at a safe volume. The easiest way to determine of your earbuds are too loud, is to set them at the volume you usually listen to and hold them at an arm’s length away. There is no cure for hearing loss, but there are treatments in the form of instruments such as hearing aids.</li><li><h4>Noise-induced hearing loss is one of the most common forms.</h4>Noise-induced hearing loss is caused by exposure to loud sounds over long periods of time. In certain occupations such as construction, factory work, and the music industry, noise-induced hearing loss is prevalent – but it can also occur due to daily exposure to unsafe volumes, such as with headphones or earbuds. The thing is, noise-induced hearing loss is 100% preventable – unlike other forms of hearing loss, such as age-related hearing loss (presbycusis).</li><li><h4>Earbuds increase the risk of hearing loss.</h4>Sure, they’re convenient and allow for mobility, but earbuds also heighten the risk of damage to your hearing. They offer almost no noise-cancellation, which means you’re listening to music and media at higher volumes than you would with over-the-ear headphones. Even with noise-canceling earbuds, their position in your ear canals, with close proximity to your ear drums, are comparable in volume to listening to power tools in a coal mine.</li><li><h4>Unplug</h4>. If you’re in a noisy environment, whether it’s on a plane or on a subway train, you may have difficulty hearing through your earbuds or headphones – unless they offer noise-cancellation. Rather than cranking up the volume – which compounds the level of damaging decibels – consider unplugging and waiting until you’re in a different environment to listen to your music. In general, it is good to unplug and give your ears a rest.</li><li><h4>Follow the 60-60 rule.</h4>Whether you’re listening over speakers or through headphones, hearing specialists recommend following the 60-60 rule: play the volume at 60% for no more than 60 minutes at a time. Again, it is important to give yourself a “noise diet” – take breaks from loud sounds frequently.</li><li><h4>Use technology to your advantage</h4>. Most devices have been designed to play at volumes much louder than recommended for hearing health. Apps have been developed to manage volume levels on your devices; consider downloading one, setting safe volume levels, and protect your hearing!</li><li><h4>Hearing loss is linked with other health problems</h4>. Individuals who experience hearing loss are at higher risk of developing depression, anxiety, and higher levels of stress. Researchers have recently found a potential link between hearing loss and dementia. The auditory system also controls your balance; individuals with hearing loss are at higher risk for falls and accidents.</li><li><h4>Consider customized ear protection</h4>. Volume levels at concerts and music festivals are extremely loud and increase the risk for hearing loss. If you’re a musician, your consistent exposure to high volumes from venue speakers endangers your hearing health. 									<p>earing loss is the third most common medical condition in the United States, affecting 48 million Americans. It is most prevalent amongst older Americans, age 65 and older. One in three people over 65, 50% of people over 75, and 80% of people over 85 experience some degree of hearing loss. Though it is a common condition, unfortunately hearing loss goes undiagnosed and untreated quite frequently. In part, this is because it is an invisible condition and it occurs gradually. For this reason, people may not be aware of their hearing loss until it has gotten progressively worse. The Hearing Loss Association of America reports that people wait an average of seven years from the time they first experience changes in their hearing to the time they decide to seek treatment. For this reason, we encourage people to begin taking annual hearing tests from the age of 50. If a hearing loss is not detected, it is still good practice to keep your hearing records on file. Hearing loss affects our ability to recognize sounds in our environment and understand speech. A common complaint among people with hearing loss is: I can <em>hear</em>, but I can’t <em>understand</em>. In other words, people with <em>untreated</em> hearing loss can hear sounds but cannot make sense of them. Sounds in their experience may be muffled or dull.  </p>								</div>

									<p><strong><u>If you have a family member with hearing loss:</u></strong></p><ul><li>Before speaking, grab your family member’s attention first. If they happen to be engaged in a task, touch their shoulder gently to grab their attention. Make sure there is eye contact so that they know you are addressing them. You could also say their name a few times until you get their attention. Keep in mind that with hearing loss – even when it is treated with hearing aids – it may be difficult to differentiate speech noise from background noise. This is especially important when you’re both in a noisy environment.</li><li>Keep in mind that there’s no need to speak louder than normal. You don’t have to raise your voice when talking to your loved one – especially if they have hearing aids. It’s more important to <em>speak clearly – </em>don’t mumble or speak too quickly. Hearing aids are equipped with excellent speech recognition features to capture what you’re saying. It also helps if you take a few pauses while talking, just in case your loved one needs to catch up.</li><li>Above all else, <em>don’t take it personally!</em> If your family member doesn’t respond or responds in a way that doesn’t actually address what you’re saying, chances are they may have misheard or misunderstood what you’ve said. Rather than getting upset, take a moment and just repeat yourself.</li></ul><p><strong><u>If you have a hearing loss:</u></strong></p><ul><li>The most important thing you can do to improve your familial relationships is to seek treatment for hearing loss. There are many benefits to seeking treatment, from improving your interpersonal relationships to reducing your risk for developing dementia.</li><li>Communicate your needs clearly to your loved ones. If you need them to slow down while speaking or sit on your right side because you can hear better out of that ear, make sure to let them know!</li><li>Wear your hearing aids! If you’ve been treated for hearing loss, make sure you regularly use your hearing aids. While they may be strange at first, over time you’ll find that they are indispensable. Hearing specialists recommend wearing your hearing aids from the time you wake up (after your bathroom routines) to the time you go to bed.</li></ul>								</div>

									<p>Our ears are certainly important apparatuses for hearing: the outer ear picks up sound and the middle ear amplifies it, with the ear drum turning sound waves into vibrations. These vibrations are then sent to the inner ear, where they are translated into neural signals. These neural signals travel well-worn pathways to the auditory processing center in the brain, where they are processed and recognized as familiar sounds. With untreated <em>sensorineural</em> hearing loss, this process is interrupted due to problems with the inner ear hair cells. Inner ear hair cells are damaged by exposure to loud noises, ototoxic (poisonous to the ear) medication, or other illnesses and injuries, such as tumors. As a result, the brain struggles to make sense of unclear sound signals, which leads to a heavier cognitive load. </p>								</div>

									<p>At Johns Hopkins University, a series of studies have indicated that untreated hearing loss leads to a heavier cognitive load. Because the brain has to exert more effort to process unclear sound signals, it must use resources designated for other cognitive functions, such as memory or concentration or balance, to process auditory data. In one study, researchers tested the cognitive abilities and hearing abilities of over 600 participants and tracked them over a span of more than 12 years. They found a direct correlation between the decline of hearing abilities and cognitive abilities. Dr. Frank Lin, a head researcher in these studies, points to an increased risk of developing dementia with untreated hearing loss. According <em>Healthy Hearing, </em>“when hearing is normal, these three areas of the brain work with the auditory system to interpret sounds and produce speech: the temporal lobe, where the primary auditory cortex is located; Wernicke’s area, located in the temporal lobe on the left side of the brain and is responsible for speech comprehension; and Broca’s area, located in the lower portion of the left frontal lobe, responsible for speech production. When hearing loss is present, the brain must work harder to make sense of the information it receives from the inner ear, which can be mentally exhausting.” <em> </em></p>								</div>

However, with hearing loss, you may experience some fatigue throughout the day, especially when you first get fitted for hearing aids and are learning to adjust. If you are in noisy environments all day, it may help to take small breaks from the noise. Find a quiet place or go for a quick walk to clear our brain. Use your lunch hour at work to go to a quiet place, read a book rather than watching a TV or movie, or use this time to meditate. You may also take out your hearing aids for a short period of time, just to relish the quiet. Meditation is a helpful tool for all people, especially those who work in stressful settings. To meditate, block out 15 to 20 minutes at first and find a quiet place to sit. Take deep breaths, and focus on your breath when inhaling and exhaling through your nostrils. If you’re not into meditation, you may also consider a power nap. The National Sleep Foundation says a 20-30 minute nap could help improve alertness and brain function.  </p>								</div>
